billing for office visits. Yvette may be contacted about
bills for services in the hospital.
Referrals
-
If you need to see a specialist and your insurer requires
a referral from the primary-care doctor, request it
here. Tell us which provider recommended the referral,
and include the child's name, the name and insurance
ID of the specialist, and the date of the appointment.
Allow one week before the appointment with the specialist.

Photos -
To
submit your photos for our web site please send
an e-mail with your attached files. JPG format is
the preferred file type. Don't forget to tell us who's
in the picture (although we will not use any last names
on the site).
Adoptions
-
If you are adopting, you may e-mail us the following
information: location of birth, when you will receive
the baby, medical information about the baby or other
family members. We will then contact you to schedule
a consultation.
